Question 1 Over the past few weeks, how often have you felt overwhelmed, anxious, or like things are "too much"?
Question 2 How much has your mood been affecting your daily life — things like work, relationships, or just getting through the day? Think about the last 2–4 weeks.
Not at allSignificantly
Question 3 Which of these have you been experiencing? (Select all that apply)
Question 4 Have you seen a therapist, counselor, or psychiatrist before?
Question 5 Are you currently taking any medication for your mental health — like an antidepressant, anti-anxiety medication, or mood stabilizer? This includes anything prescribed by a doctor or psychiatrist for mood, sleep, or emotional health.
Question 6 Have you ever felt like weekly therapy alone wasn't enough to help you get through what you were going through?
Question 7 Have you noticed that your mood or emotions seem to shift around your period or menstrual cycle? For example: feeling more anxious, irritable, tearful, or low in the days before your period.
Question 8 Have you been diagnosed with or do you suspect you have PCOS (Polycystic Ovary Syndrome) or Endometriosis? These are hormonal conditions that can affect mood, energy, and emotional wellbeing in ways that are often under-recognized.
Question 9 Have you had hormone levels or labs checked in relation to how you're feeling emotionally? This could include things like thyroid levels, estrogen, progesterone, testosterone, or vitamin D — all of which can impact mood and energy.
Question 10 Are you currently pregnant, or have you given birth in the last 2 years?
Question 11 If you've been pregnant or given birth recently — have you experienced any of the following? (Select all that apply) If not applicable, just select the last option.
Question 12 Are you between the ages of 35–65, and have you noticed shifts in your body or mood that might be related to hormonal changes? Perimenopause can start in your late 30s or 40s and often causes mood changes that get mistaken for depression or anxiety.
Question 13 Have you experienced any of these lately? (Select all that apply) These can be signs of hormonal shifts that affect emotional health.
Question 14 Have you been through something really difficult in your past that still affects how you feel or move through the world today? This might include a difficult childhood, a loss, an abusive relationship, or a traumatic experience. You don't have to name it here.
Question 15 Do you find yourself repeating the same emotional patterns — reacting in ways you wish you could change, or feeling stuck in the same cycles?
Question 16 Right now, how much support do you feel like you need?
Question 17 Would you be open to a program that meets 3 days a week for a few months, combining group sessions with individual therapy? Our IOP involves coming in several times a week, giving more consistent support than traditional weekly therapy.
Question 18 Would it feel helpful to go through treatment alongside other women who are dealing with similar things? Our program is women-only, which many find allows for more openness and safety.
Question 19 Have you ever felt like your doctor or providers weren't really listening to you, or told you things were "fine" when you knew something wasn't right?
Question 20 What would mean the most to you in a program like this? (Pick your top priority)
